Purpose 1.1. The Department is required to make a prompt and thorough investigation of reports of child abuse or neglect allegedly committed by a person responsible for a child's care, custody, or welfare, pursuant to §261.301, Texas Family Code. 1.2. The Department, for the reasons shown below and in an affidavit attached hereto and incorporated herein in its entirety, requires court orders authorizing the Department to: 1.2.1. Obtain **CHILD3**'S prior medical, psychological or psychiatric records; and/or 1.2.10 Obtain **CHILD4**'S prior medical, psychological or psychiatric records; and/or 1.2.11 Obtain a medical, psychological, or psychiatric examination of **CHILD1** pursuant to § 261.303, Texas Family Code. 1.2.12. Obtain a medical, psychological, or psychiatric examination of **CHILD2** pursuant to § 261.303, Texas Family Code. 1.2.13. Obtain a medical, psychological, or psychiatric examination of **CHILD3** pursuant to § 261.303, Texas Family Code. 1.2.14. 2. Facts 2.1. On DATE , the Department received a report of child abuse and/or neglect with respect to the above-referenced child. 2.2.
